By submitting this survey, I ("Client") acknowledge and agree to the following terms regarding my testimonial, images, and any related content shared with Lumen Optimal Wellness ("Company").

1. Consent to Use
I grant Lumen Optimal Wellness the irrevocable right to use my testimonial, written responses, and any images I upload, including my likeness, before/after photos, or other submitted media, for marketing and promotional purposes. This includes, but is not limited to:

• The Lumen Optimal Wellness website (lumenoptimalwellness.com)

• Social media platforms (Instagram, Facebook, etc.)

• Email newsletters and client communications

• Printed materials, including brochures, posters, and in-office displays

• Advertising campaigns, including online and physical advertisements

2. Usage Rights
I understand that:

My testimonial and photos may be edited for clarity, length, or formatting while maintaining the intent of my words.
Lumen Optimal Wellness will not sell or transfer my testimonial or images to third parties for unrelated marketing.
I am not entitled to any compensation beyond the stated promotional credit ($10 account credit) for my submission.

3. Revocation of Consent
I understand that I may request to withdraw my testimonial and/or images from future use by emailing info@lumenoptimalwellness.com. However, I acknowledge that previously published materials may not be retractable.

4. Privacy & Confidentiality
Lumen Optimal Wellness will only use my first name and last initial (e.g., "Sarah D.") or an alternative identifier if requested. Personal contact details will remain confidential and will not be shared publicly.

5. Confirmation of Original Content
I confirm that any photos or content I submit are my own, and I have the right to grant this permission. I understand that unauthorized use of third-party images or text is prohibited.
